Marketing Script / Cookie Intake

Please fill out the form below to formally request the placement of a script or cookie for marketing purposes (campaign, tracking, etc.). Submitted requests will flow to the GIS Digital team for further review. For more information on the review process please refer to the Marketing Scripts Standard Operating Procedure.

*All requests will be approved or denied within a one week period. If there is any reason to extend this approval window you will be notified within that first week. 
*All approved requests will have a minimum of one to two weeks turnaround on implementation. This date will be provided once approved.
Who is filling out the request form
Owner responsible for the campaign/tag/pixel within Owens Corning
e.g. Interrupt, Brunner, etc. Please enter N/A if internal and no third-party is involved
Place the requested script here and any further details necessary about the implementation
What is the budiness intending to accomplish through this implementation? e.g. General tracking of data, increase in revenue, etc.
Personal data means the script will push data that will be able to be used to identify someone as an individual (e.g. name, email, etc.). This is a rare occurence
This would be yes if it is capturing any data or activity that will later be used to reach out to the same users in a future campaign (e.g. retargeting)
Please provide at least vendor name (ex. Facebook)
